POLICIES AND INSURANCE
BOOKING AND CANCELLATION POLICIES
- Payment is due at the time of service. Payment by debit card, credit card, HSA/FSA card, cash, and check is available.
- Be Happy Family Chiropractic requires 24-hour notice for any cancellations or changes to your appointment. Patients who provide less than 24-hour notice will be subject to a $25.00 cancellation fee.
- NEW PATIENTS who miss OR no-show their appointment in its entirety will be subject to a $50.00 deposit if they wish to re-schedule their appointment. This non-refundable deposit will be used to cover part of the first visit fee. If a patient wishes to re-schedule their visit, a new $50.00 deposit will be required.
- Established patients who miss OR no-show their appointment in its entirety will be subject to a $45.00 missed appointment fee.
INSURANCE OPTIONS
- MEDICARE (TRADITIONAL and RAILROAD) accepted.
- At this time, we are not In-Network with any other insurances. We are a self-pay facility and collect payment at the time of service. Super-bills (itemized bills) are available upon request if you wish to personally submit them to your insurance or FSA/HSA accounts - keep in mind that reimbursement is not guaranteed. This allows us to better serve you and create a specific care plan tailored to your health status and goals.
